Entry Procedure
1. Complete the form below.( Please only use this form for prepayment before the day, a different marked registration form is issued with on the day payment.)
2. Enclose a cheque payable to "Lions Club of Fleet" for the entry fee of £8.00 'before the day' (entry is £10.00 in cash on the day)
3. Print this page, sign and return it with the entry fee to the Secretary at the address below by April 19th Please do not send S.A.E. as number, route sheet, badge etc are issued on the day

I hereby accept the conditions of entry. I do have full insurance cover. I am happy for Fleet Lions to keep my details for information about this event and related matters in the future. Your details will not be passed on to anyone else.
INDEMNITY:
I have read the Regulations for the Event and I agree to be bound by them to save harmless and keep indemnified the Organisers from and against all actions, costs, expenses, claims and demands in respect of injury, fatal or otherwise, loss or damage, to the persons or property of myself or of my passenger or any other person, howsoever caused or arising out of or in connection with my entry or my taking part in this meeting, and whether or not occasioned or contributed to by reason of the negligence of the said bodies, officials, representatives or agents. My insurance covers an event of this kind.
